A Four-Location Dental Group Cut No-Shows From 18% to 6%
A dental group was losing an estimated $40,000 a month to missed appointments. A layered AI reminder and rebooking workflow brought the no-show rate down by two-thirds.
An 18% no-show rate sounds high until you look at what it costs a multi-location practice month over month, and this group had already tried the obvious fix.
Why one reminder wasn’t enough
The practice had a single 24-hour-out text reminder running for over a year with no meaningful change in no-show rate. The assumption going in was that patients simply weren’t seeing it. The real issue was different: patients were seeing it and still not acting on it, because a single passive reminder doesn’t create urgency or make rescheduling easy in the moment.
Building the sequence
We built a three-touch sequence instead of a single message. The 72-hour text includes a one-tap confirm or reschedule link, so patients who know they can’t make it can move the appointment immediately instead of waiting to cancel last minute. The 24-hour text repeats this for anyone who hasn’t responded. For the small group who still haven’t confirmed by the morning of, a voice call goes out, since some patients, especially older ones, respond to a phone call in a way they don’t respond to text.
Automating the recovery, not just the reminder
The bigger shift was what happens after a no-show. Instead of a staff member calling to try to rebook, sometimes days later, the system sends a same-day rebooking link automatically. Patients who no-show but were going to reschedule anyway now do it themselves within hours instead of the practice chasing them down.
Results
No-shows fell from 18% to 6% within two months across all four locations, and stayed there through the following quarter. The practice’s retainer now covers quarterly tuning of the sequence timing as patient response patterns shift by season.
Does this comply with healthcare communication regulations?
Yes. The workflow was built to work within the practice's existing HIPAA-compliant patient communication platform rather than introducing a new tool, and all patient data stays inside that system.
Why did a second reminder plus a call work better than just texting more often?
Different patients respond to different channels. The 72-hour text catches people who plan ahead, the 24-hour text catches people who don't check messages until the day before, and the morning call reaches the smaller group who don't respond to text at all. Layering them, instead of just sending more texts, is what closed the gap.
How much staff time did this actually save?
Front desk staff estimated 6 to 8 hours a week previously spent manually calling no-shows to rebook. That time is now spent on patients who are already at the front desk, which the practice manager considers the bigger win.
